How Do I Create a Multi-list Form?

To create a multi-list form, do the following:

1. Go To Contacts

2. Choose the Multi-List Options tab and the default sub tab you will see is Multi-List Forms.

3. Click on Add A Multi-List form button
   

4. Next, you will see a listing of lists available in your system.  We also provide you with some information on how to create a single list form.

5. Chose the lists that you would like to include in the form and click on the Go Next button.
   

6. Give your multi-list a name so that you can identify them later on.

7. By default, you will see the Name and Email Address already created for your form.

8. To add more fields for your form, click on Add Field.
   

9. Change the New Field text to your preferred text.

10. Next, choose which column you would like to map the new field to from the drop down field.  For example, if you name your field as Telephone Number, you should map to office number, home number or mobile number.

11. By default, we display only 2 lists.  To view more lists, click on the arrow button to scroll to the left or right.

12. You can preview what your form looks like under Form Preview area and this preview will dynamically update itself when you make changes to the form.

13. If you would like to rearrange your form, you can click on the form field and drag it up or down and release it on the row that you want the field to be in.   At the same time, your form preview will be updated showing you the latest sequence of your form fields.

14. If you would like to remove any of the additional fields that you have created, simply click on the X button (this is the Remove button), followed by the Remove This Field link.

15. Next, tell us how we should direct your form Success Page, Error Page and Activation Page.  We provide you with a default page for all of them (you can view them by clicking on View our default success page) but if you would like to use your own page, enter the URL in the field provided.

16. Once everything is looking good, you can click on the Save Form button and we will bring you to a page where you can copy the form code.
    

We provide you with two options to put your form on your website.

1. Put the form code on your own webpage

   1. Copy the form code and paste it onto your webpage.  If you are not familiar with this, please contact us and we can assist you with this for a nominal fee.

   2. Now you are ready to accept subscribers for your email campaign

2. Host the form on our system
   1. If you would like us to host your form on our system, you can simply copy the link provided under Host the form on our system and put it on your webpage.
   2. When users click on the link, they will be brought to your form hosted on our system.
   3. Now you are ready to accept subscribers for your email campaign
